ICM: Intensive Case Management Description: • ICM services are provided for current and former children in state care youth ages 16-21. • Target youth are seniors in high school and youth emancipating from the foster care system. • The goal of ICM is to provide training, support, and guidance to youth emancipating from foster care that will enhance their ability to successfully transition into independent living.
